Please use the form and drop-down menu to SEND PATIENT RECORDS to one of The LifeCentre practices.

• Please be specific to ensure the records are sent to the correct specialty practice or our emergency department
• Include all the medical records and notes, radiographs, bloodwork and test results available
• If you have questions, please call the practice (Cardiology, for example)  directly to ensure you send everything they’ll require for your referral.
