About Referrals to The LifeCentre Practices

• The LifeCentre practices will only provide the services for which a patient has been referred
• Each practice may need different forms or records for a referral
• Please send us records via the Send Us Records page and select the practice from the drop-down menu
• There are links to their forms and requirements so you can submit records directly to them.  There is also a window to include a message with your records.
• If a question should arise during diagnosis or treatment, our team will call you to discuss the case, and together, determine the best course of action for the patient and pet owner
• If as a result of their visit, we determine that it is in the pet’s best interests for an interdepartmental transfer, we will discuss it with your client and notify you and your team
• Since we are a multi-specialty center, this is easily and conveniently accomplished with all records and results forwarded to you
• Should you have any questions or concerns, our doctors are available and happy to discuss their findings and recommendations as we are part of your patients care team, and value your insight as their primary care veterinarian
• Appointments for consultations can be made by you on behalf of your client – or – by the client upon your referral
• You do not need to call TLC when making a referral; however, your inquiries are always welcome, and details regarding a patient are always appreciated
• We appreciate receiving all pertinent medical records, diagnostic test results, and a completed referral form prior to the consultation
